6/16/2023 EnchantaVaughn Approved QMHP-A greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHP-A inquired about her morning. Kaila verbalized to QMHP-A that her morning was going well. QMHP-A informed Kaila that this session was going to continue to focus on managing her depressive symptoms. Upon arrival, she did not present any symptoms of S/I. QMHP-A began the session by inquiring about Kailaâs current thoughts, feelings, and emotions. QMHP-A engaged Kaila in a discussion surrounding her medication management. QMHP-A discussed with Kaila the importance of scheduling important medical appointments and attending them and not cancelling them on a regular basis. QMHP-A discussed with Kaila she is to attend all schedule appointments without any excuses. QMHP-A engaged Kaila in a skills building activity. QMHP-A informed Kaila that the purpose of this activity was for QMHP-A to guide Kaila through a series of questions where she will identify signs of depression and situations that causes her to become depressed. QMHP-A questioned Kaila about different techniques she applied to overcome depression and recommended she apply those methods now. QMHP-A engaged Kaila in a discussion surrounding her ADL's and IADLâs. QMHP-A conducted a skills building activity with Kaila. QMHP-A informed Kaila that the purpose of this activity was to assist her keeping up with her chores and planning. QMHP-A also educated Kaila that the second part of this skills building activity was to so that she could write down her daily activities and chores to display her activities of daily living. QMHP-A demonstrated with Kaila adult living skills through role play. QMHP-A encouraged Kaila about improving personal hygiene regiments. Towards the end of the session, QMHP-A assist Kaila with how she is going to budget her monthly income for the upcoming month being as though she is about to move into another apartment within the next two weeks. QMHP-A conducted a skills building activity with Kaila to assist her with this achieving this goal to include QMHP-A providing her with a budget sheet. Kaila appeared to be receptive to QMHP-Aâs interventions and services that were provided to her during the session. Kaila discussed with QMHP-A that she thinks that she has been slacking when it comes to cleaning her home and other things that needs to be done in her home. Kaila indicated to QMHP-A that she has her focus surrounding her daughter and the trifling things that she does that her mind is not where it needs to be. During the skills building activity, Kaila was able to understand how planning will assist her with getting chores completed in her home on a daily basis.
